Referrers

Rowanbrook Care Home in Walsall works closely with healthcare professionals, local authorities and commissioning teams to support adults with complex care needs.

Our experienced multidiscipline clinical team, including registered nurses, clinical care practitioners and therapeutic support staff, collaborates with social workers, hospital discharge teams, case managers, NHS Continuing Healthcare assessors and local authority professionals understands the importance of careful assessment and placement planning. We work collaboratively with referrers to ensure every admission is appropriate and that residents receive the support they need from the moment they arrive.

Making a Referral

Referrers are encouraged to contact the home to discuss potential placements and provide background information about the individual’s care needs. Our team reviews this information carefully to determine whether Rowanbrook is likely to be an appropriate environment for the individual.

Following the initial enquiry, a detailed clinical assessment is arranged. This assessment may take place within the individual’s current setting, such as a hospital ward, their home or another care environment. The assessment allows our clinical team to understand the individual’s physical health, mental wellbeing and support needs.

Once the assessment is complete, we work with the referrer and relevant professionals to plan a suitable placement and develop a personalised care plan. When a placement is confirmed, our team coordinates the admission process to ensure the transition into the home is smooth and supportive.

Our team are highly experienced, compassionate and caring, who strive to deliver the best possible care to those living with dementia. They understand the complexities of the many different subtypes of dementia such as Alzheimer’s disease, vascular dementia, frontotemporal dementia and other rare types of dementia.

Our focus is to understand the uniqueness of the individual and to understand the impact of the progression of the illness, to continually support and enhance wellbeing. We also recognise the impact a diagnosis of dementia can have on family members and our care and support extends to this wider network

Rowanbrook Care Home accepts referrals for adults who require specialist nursing or residential care due to dementia, complex physical health needs or mental health conditions.

Our clinical team aims to review referrals promptly and arrange assessments as quickly as possible, particularly when individuals require timely discharge from hospital or urgent placement.

Referrers are encouraged to provide relevant clinical documentation such as care plans, risk assessments, medical summaries and medication information to support the assessment process.
